This service is increasing the influence of people with lived experience of multiple disadvantage on the design and delivery of services and the city’s overall strategy and approach to multiple disadvantage.
Our Core Team has a caseload of adults experiencing multiple disadvantage and provides them with an enhanced offer of support.
Staff in the core team work citywide with small caseloads to allow for intensive support over the life of the programme.
We partnered with the Sheffield Drug and Alcohol Commissioning Team to co-commission the 'Positive Activities for Adults in Recovery Fund' which is being administered and coproduced by South Yorkshire Housing Association
Project 6 deliver a training service to raise awareness of multiple disadvantage and implementing trauma-informed and person-centred approaches.
We will use learning from all areas of the programme at a system level to develop consensus and sustainable, collective solutions.
Changing Futures and the Integrated Care Board increased the capacity of Sheffield Health and Social Care NHS Trust's Homeless Assessment and Support Team which provides access to healthcare services.
We have partnered with Paradigm Psychology to provide psychological support to our staff.
We use a mixture of data-led and operational intelligence approaches to inform how we work, from identifying our caseload to helping inform service design and delivery.
